The operator is being identified as Staci Jackson (56) of Eugene.

On Tuesday, June 8, 2021 at approximately 11:48 P.M., Oregon State Police Troopers and emergency personnel responded to a single vehicle crash on Hwy 126E near milepost 17.

Preliminary investigation revealed a Ford Escort was westbound when it left the roadway and struck a power pole.

The operator was transported to Riverbend Hospital where she was pronounced deceased.  The name of the operator will be released when appropriate.

OSP was assisted by Emergency Medical Services and ODOT 

Fatal Crash on Hwy 199 – Josephine County06/08/21

On Monday, June 7, 2021 at approximately 2:30 P.M., Oregon State Police Troopers and emergency personnel responded to a two vehicle crash on Hwy 199 near mile post 25.

Preliminary investigation revealed a Dodge Caravan, operated by Ronald Johnson (73) of Cave Junction, was southbound when it hit the back of a Nissan Frontier, operated by Brandi Millen (39) of Kerby, which was stopped to turn left into a private driveway.  

Johnson sustained fatal injuries and was pronounced deceased while being transported by ambulance. 

Millen was transported to the hospital with non life threatening injuries. 

OSP was assisted by the Illinois Valley Fire District and ODOT. 

Fatal Crash on Hwy 138E – Douglas County06/04/21

On Thursday, June 3, 2021 at approximately 5:30 P.M., Oregon State Police Troopers and emergency personnel responded to a single vehicle crash on Hwy 138E near milepost 28.

Preliminary investigation revealed a Ford Taurus, operated by Adam Sherman (47) of Roseburg, was westbound when it left the roadway and struck a tree.

Sherman sustained fatal injuries and was pronounced deceased.

A juvenile passenger was transported by air ambulance to the hospital.

OSP was assisted by Glide Fire Department, Umpqua Valley Ambulance, and ODOT

Fatal Crash on I-84 – Umatilla County06/03/21

On Wednesday, June 2, 2021 at approximately 3:30 P.M., Oregon State Police Troopers and emergency personnel responded to a vehicle crash on Interstate 84 near milepost 206.

Preliminary investigation indicates a Dodge Ram pickup pulling a travel trailer, operated by Kurt Hitchen (48) of Pendleton, was eastbound when the trailer began to sway causing both vehicles to roll.  

Hitchen sustained fatal injuries and was pronounced deceased.

Passenger Melissa Hagen (38) of Pendleton was transported to St. Anthony’s Hospital in Pendleton.

OSP was assisted at the scene by the, Pendleton Fire Department, Echo Fire Department, and ODOT.

UPDATE – Fatal Crash on Hwy 58 – Lane County05/31/21

Oregon State Police is requesting the public’s assistance with witnesses to yesterday’s triple fatality crash.

A fourth vehicle, possibly a blue pickup was involved in the crash but left the scene without providing information.

Names of operator and occupants of Mazda M3

Mazda M3 was operated by Jagadish Chandrasekaren (31) of Washington sustained fatal injuries and was pronounced deceased.

Passengers:

Adharsh Murali (25) of Washington sustained fatal injuries and was pronounced deceased.

Jignesh Modi (27) of Washington sustained fatal injuries and was pronounced deceased.

Puneeth Gattikoppula (24) of Washington transported by air ambulance to the hospital with serious injuries.

OSP is/was being assisted by the Lane County Sheriff’s Office, Oakridge Fire Department, Goshen Fire Department, Dexter Fire Department, Lane County District Attorney’s Office, and ODOT.

On Sunday, May 30, 2021 at approximately 10:15 A.M., Oregon State Police Troopers and emergency personnel responded to a vehicle collision on Hwy 58 near milepost 27.

Preliminary investigation revealed a Ford Taurus, operated by James Johnson (31) of Oakridge, was westbound passing in a no passing zone when it collided with a also westbound Chrysler Town and Country, operated by Michael Cary (63) of Oakridge, the Taurus then collided with a eastbound Mazda 3. 

Three occupants in the Mazda 3 sustained fatal injuries and were pronounced deceased.  A fourth occupant was transported by air ambulance to the hospital with serious injuries.

Cary was not transported but later went to the hospital for treatment.

Johnson was transported to the hospital.

Investigation is continuing and names will be released when appropriate.

Fatal Crash on Hoover Creek Lane – Wheeler County05/30/21

On Saturday, May 29, 2021 at approximately 4:20 P.M., Oregon State Police Troopers and emergency personnel responded to a vehicle crash on Hoover Creek Lane outside of Fossil.

Preliminary investigation revealed a Honda 450R motorcycle, operated by Timothy Binder (32) of Condon, was northbound and collided with a southbound Subaru Outback operated by Thomas Edwards (71) of Bend.

Bender sustained fatal injuries and was pronounced deceased.

Edwards and passenger, Gale Brown (61) of Bend, received minor injuries.

OSP was assisted by Wheeler County Sheriff’s Office and EMS. 

Fatal Crash on Hwy 97- Klamath County05/30/21

On Saturday, May 29, 2021 at approximately 8:22 P.M., Oregon State Police Troopers and emergency personnel responded to a single vehicle crash on Hwy 97 near mile post 173.

Preliminary investigation revealed an Audi All-Track, operated by Richard Enquist (41) of Bend, was southbound when it went off the roadway and rolled.

Enquist sustained fatal injuries and was pronounced deceased.

OSP was assisted by the Deschutes County Sheriff’s Office, Lapine Fire Department, ODOT, and Central Oregon Police Chaplaincy.

Fatal Crash on Hwy 206 – Sherman County05/29/21

On Friday, May 28, 2021 at approximately 3:20 P.M., Oregon State Police Troopers and emergency personnel responded to a single vehicle crash on Hwy 206 near milepost 12.

Preliminary investigation revealed a Harley Davidson motorcycle, operated by Ricky Stocker (62) of Hillsboro, was eastbound when it went off the road and down an embankment.

Stocker sustained fatal injuries and was pronounced deceased.

OSP was assisted by the Sherman County Sheriff’s Office, Sherman County EMS, and ODOT.

Fatal Crash on Hwy 20 – Lincoln County05/24/21

On Monday, May 24, 2021 at approximately 9:44 A.M., Oregon State Police Troopers and emergency personnel responded to a multiple vehicle collision on Hwy 20 near milepost 3.

Preliminary investigation revealed a Ford F250 truck towing a 16 foot fully enclosed trailer, operated by Devin Shrum (32) of Lebanon, was eastbound when it lost control crossed into the westbound lane and collided with a Ford Fiesta operated by Kayla Rhoades (29) of Toledo. 

The trailer came loose and struck a westbound Dodge Magnum operated by Jeremy Lacewell (43) of Toledo. 

Rhoades sustained fatal injuries and was pronounced deceased. 

Shrum and his passenger, Lonnie Shrum (64) of Lebanon, were transported to Samaritan Pacific Hospital.

Lacewell and his passenger, Jimmy Short (36) of Toledo, were not injured.

OSP was assisted by Lincoln County Sheriff’s Office, Toledo Police Department, Newport Fire Department, Toledo Fire Department and ODOT.

Fatal Crash on Hwy 101 – Lincoln County05/20/21

On Wednesday, May 19, 2021 at approximately 11:24 pm, Oregon State Police Troopers and emergency personnel responded to a two vehicle crash on Hwy 101 near milepost 128.

Preliminary investigation revealed a Ford F350, operated by Jesus Perez (25) of Salem, was northbound when it crossed into the southbound lane and collided with a Kia Spectra operated by George Linden III (46) of Toledo.

Linden sustained fatal injuries and was pronounced deceased.

Perez was transported to Salem Hospital.

OSP was assisted by the Lincoln County Sheriff’s Office, Depoe Bay Fire Department, and ODOT.     

UPDATE #3- Officer Involved Shooting Investigation – Marion County05/19/21

The 17 year old victim was pronounced deceased at Doernbecher Children’s Hospital on May 19, 2021.

Peden is scheduled to be arraigned on indictment Thursday, May 21, 2021 at the Marion County Circuit Court Annex.

Any further inquiries should be directed to the Marion County District Attorney’s Office.  As this is an open criminal prosecution, no further information will be released at this time.

Yesterday’s officer involved shooting incident began at approximately 4:45 P.M., when Gervais Police Department responded to a disturbance in the 1000 block of Depot Ct. in Gervais.  Upon arrival they located Arik Reed (22) of Gervais with a gunshot wound.  Reed was transported to Salem Hospital.  Officers were able to determine that Kenneth Peden III had kidnapped a 17 year old female after the shooting and left the area in a Ford F-150. 

An attempt to locate was broadcast to all Marion County law enforcement and, shortly afterward,  a Marion County Sheriff’s Deputy located the pickup occupied by Peden and the female kidnap victim.  Peden fled from law enforcement when they attempted to stop his vehicle.  The pursuit continued to Hwy 214 at about mile post 37, where the officer involved shooting event reported yesterday occurred.

OSP is/has been assisted by Gervais Police Department, Mt. Angel Police Department, Woodburn Police Department, Salem Police Department, Keizer Police Department, Silverton Fire Department, ODOT and the Marion County District Attorney’s Office.

Officer J.J. Lamoreaux, Silverton Police Department, 14 years in law enforcement.

Officer Braden Anderson, Silverton Police Department, 1 year in law enforcement.

The male driver of the F-150, Kenneth Williams Peden III (20) of Woodburn, was arrested and lodged at the Marion County jail on the following charges:

1.            Attempted Murder in the First Degree with a Firearm 

2.            Attempted Murder in the Second Degree with a Firearm 

3.            Unlawful Use of a Weapon with a Firearm 

4.            Kidnapping in the First Degree

5.            Fleeing and Attempting to Elude by Vehicle

On Wednesday, on May 12, 2021, law enforcement officers from the Marion County Sheriff’s Office and the Silverton Police Department became involved in a vehicle pursuit of a possible kidnapping suspect.  While in pursuit of the white Ford F150, an occupant in the involved vehicle fired multiple rounds toward police as they traveled through the city of Silverton.  The vehicle finally came to a stop on Highway 214 near Forest Ridge Road NE where the suspect exchanged gunfire with involved officers. 

The male driver of the Ford F-150 was detained by law enforcement; the female passenger was transported by Life Flight from the scene with life threatening injuries.  Multiple police vehicles were struck by gunfire during the incident, no law enforcement officers were injured.

A Marion County Sheriff’s Office Deputy and two Silverton Police Officers were involved in the shooting. Pursuant to Marion County Senate Bill 111 protocols, the Oregon State Police will be leading the investigation into the incident and the involved law enforcement officers will be placed on Administrative Leave.

UPDATE – Fatal Crash on Interstate 5 – Linn County05/18/21

The operator of the Honda Civic – Bradley Letsom (33) of Eugene.

On Sunday, May 16, 2021 at approximately 10:30 P.M., Oregon State Police Troopers and emergency personnel responded to a vehicle crash on Interstate 5 near milepost 208.

Preliminary investigation revealed a Honda Civic was the subject of several complaints for poor driving northbound on I-5.  As troopers were still responding to the area the Civic turned around and proceeded southbound in the northbound lanes.  The Civic collided with a northbound semi-truck operated by Surjit Singh (38) of Live Oak, CA.

The operator of the Civic sustained fatal injuries and was pronounced deceased.  The name will be released when appropriate.

Singh was not injured in the crash.

OSP was assisted by Linn County Sheriff’s Office, Harrisburg Fire Department, and ODOT.

Fatal Crash on Hwy 97 – Klamath County05/17/21

On Monday, May 17, 2021 at approximately 2:25 P.M., Oregon State Police Troopers and emergency personnel responded to a single vehicle crash on Hwy 97 near milepost 269.

Preliminary investigation revealed a Smart Fortwo car, operated by Deborah Seaquist (67) of Chiloquin, was northbound when it went off the roadway, struck a rock and rolled.

Seaquist sustained fatal injuries and was pronounced deceased.

OSP was assisted by Klamath County Fire District #1 and ODOT.

Oregon State Police Investigating Shooting – Klamath County05/17/21

On Saturday, May 15, 2021, at approximately 10:02 A.M., Klamath County 911 received a call from Jeremiah Cogburn (36) reporting he had gotten into a fight with his friend.  He had punched his friend during the fight and his friend had, “took off into the hills.”

At approximately 10:15 A.M., Klamath County 911 received a second call on the incident.  A woman reported her 23-year-old nephew had been shot in the head and was being transported to the hospital.

Klamath County Deputies contacted Cogburn at his residence on Canyon Drive in Chiloquin.  He was brought to Klamath Falls, where he was interviewed by Oregon State Police Detectives.

The victim was contacted at Sky Lakes Medical Center where he was interviewed by a Klamath Falls City Police detective.  The victim reported he was “pistol-whipped” on his head by Cogburn, prior to Cogburn shooting him. 

The victim received an injury on his head, apparently from the bullet grazing the top of his head.  The victim also had several other injuries on his head that required medical attention.

Cogburn was taken into custody and lodged in the Klamath County Jail. 

He was charged with Attempted Murder, 2 counts of Assault in the Second Degree, 2 counts of Unlawful Use of a Weapon, Menacing and Recklessly Endangering.

Serious Injury Crash on Hwy 26 – Jefferson County05/17/21

On Sunday, May 16, 2021 at approximately 9:50 P.M., Oregon State Police Troopers and emergency personnel responded to a vehicle crash on Hwy 26 near milepost 97.

Preliminary investigation revealed a Dodge Dakota, operated by, Benjamin Davis (20) from Bend, was eastbound attempting an unsafe pass on a Subaru Legacy operated by Marlon Pendleton (69) from Bend.  The Dakota collided with a westbound Toyota Camry, operated by Richard Sixto-Tello (22) from Sandy, and then collided with the Subaru Legacy.

Davis was transported by ground ambulance to St. Charles Bend. 

Sixto-Tello was transported by Life Flight to St. Charles Bend.

Pendleton was transported by ground ambulance to St. Charles Madras. 

Upon release from St. Charles Bend, Davis was taken into custody and transported to the Jefferson County Jail where he was cited for Assault III, Assault IV, DUII, Reckless Driving and Reckless Endangering.

Fatal Crash on Hwy 395 – Umatilla County05/16/21

On Saturday, May 15, 2021, at approximately 4:10 P.M., Oregon State Police Troopers and emergency personnel responded to a motorcycle crash on Hwy 395 near milepost 53. 

Preliminary investigation revealed that a BMW motorcycle, operated by Steven Orchekowsky (53) of Newcastle, WA., was northbound when it left the roadway and crashed.

Orchekowsky sustained fatal injuries and was pronounced deceased.

OSP was assisted by Ukiah Quick Response Team and ODOT.

Several Beaches still closed to razor clam digging- Northern Oregon Coast05/13/21

The Oregon State Police and other local law enforcement are taking enforcement action against unauthorized razor clamming along the Northern Oregon Coast particularly in the popular Clatsop Beach area.  Razor clamming remains closed from the north jetty of the Siuslaw River to the Columbia River (including inside the Columbia River) due to high domoic acid levels.  This is basically the coastline from Florence to Astoria. Mussel, bay clam, and crab harvesting remain open along the entire Oregon coast. Coastal scallops are not affected by biotoxin closures when only the adductor muscle is eaten. ODA does not recommend eating whole scallops. Commercial shellfish products remain safe for consumers.

Paralytic shellfish toxins and domoic acid toxins are produced by algae and originate in the ocean. ODA will continue to test for shellfish toxins twice per month, as tides and weather permits. Reopening an area closed for biotoxins requires two consecutive tests with results below the closure limit.
